HCS Service Coordination
Every consumer in our facility is assigned to a coordinator who coordinates the development and implementation of each person’s Individual Plan of Care and Individual Services Plan, integrates various aspects of services provided, advocates waiver and non-waiver services and record each person’s progress.
RESIDENT ASSISTANCE
We offer individuals enrolled in our program the options to choose among the following Residential assistance types
Supported Home Living
Services are provided to individual residing in their own residence or residence of their Natural or adoptive parents
HCS Foster/Companion Care
Services provided to individuals in a residence of a foster or companion care provider exclusive of Texas Department of Protective and Regulatory Services and Texas Department of Human Services.
Residential Support
Services provided to individuals by daily shift staff or by an individual staff member with periodic relief staff.
Respite
Respite services are available to individual residing in their own residence or residence of their natural or adoptive parents. Respite Services may also include emergency and/or temporary residential services. Consumers may receive respite services up to 30 days each year.
Day Habilitation
Our consumers are provided quality day habitation services designed to assist individuals in acquiring, retaining and/or improving self-help, socializing skill and adaptive skills necessary to reside in home and community based settings. Services are designed to meet individual needs.
Supported Employment
Individuals under legal retirement age and determined by individual need may receive supported employment to facilitate community employment based on:

1. Employment in an integrated work setting away from the individual’s residence
2. Provision of ongoing individualized supported needed to sustain paid work by the individual including supervision and training
3. Compensation by the employer to the individual in accordance with Fair Labor Standard Act.
4. Provision of services not available or funded through the state education agency or the state rehabilitation agency.
Nursing
Our agency provides nursing services to ensure the general health and well being of our consumers. Nursing services include monitoring the individuals’ health and medical conditions, administration of medications, making referral and providing medical treatment as ordered by physician.
Dental Services
Consumers are eligible for dental services and determined by individual needs. Dental Services include:
- Emergency dental treatment
- Preventing dental treatment
- Therapeutic dental treatment and
- Orthodontic dental treatment excluding
- Cosmetic Orthodontia
Counseling and Therapy
Our agency provides an array of counseling and therapy services that include physical therapy services that include physical therapy occupational therapy, speech therapy, audio logy and social work. We provide psychological services that include individualized plan designed to modify, develop and improve the cognitive, social and effective skills. We also provide services that are designed to meet individual nutritional needs.
Adaptive Aids
Adaptive aids includes devices, controls or appliances which enables person to retain or to increase their abilities to person activities of daily living or control the environment in which they live
Minor Home Modification
Minor home modifications include structural modification to the consumer’s home to improve mobility or access depending on the individual’s needs.
Amazing Grace Healthcare Services is a state certified and licensed angency dedicated to proving developmental assistance to individuals with mental retardation or related conditions. We believe that services must be provided in accordance with principle of normalization and our ultimate goal is to assist individual with mental retardation to live independent life.
